Note: Since you need to print the survey confirmation page, be sure that you have access to a printer on the computer you use to take the survey. You will also need to know your University ID (top right corner of your UCARD) and the FYS class you are enrolled in since these are required data entry fields on the survey.

The First Year Seminar beginning survey is taken by all students registered for a First year Seminar Course. This survey asks you a variety of questions about your college and high school experiences. The results of this survey will provide IU Southeast with information to enhance the types of experiences you may have while at college.

The survey consists of 8 pages and should take about 30 minutes to complete. Please allow yourself sufficient time to complete the survey.

Pages 1 – 7 of the survey are the actual survey questions. You must answer ALL questions on the first page. They are identifying questions and you cannot continue through the survey without answering them. Once you complete the first page, you may skip any question in the survey (pages 2 – 7) that you do not want to answer. It is important that you answer the questions honestly.

Please read page 8 of the survey carefully before answering the question at the bottom of the page. The question asks whether or not you wish to participate in a research study. Your participation in this study is voluntary. If you decide not to participate, information will be used only to assess the First Year Seminar course and not for research purposes. There will be no penalty for not participating. If you decide to participate, you may withdraw from the study at any time until the identifiers have been removed from the data set. A response to the question on this page is required.

Once you have completed the entire survey, click the last arrow >> button to submit your survey. Once your survey has been successfully submitted, you will be taken to the confirmation page displaying a thank you message along with your UID. If you do not reach the confirmation page, then you have not completed the survey and by closing your internet browser prematurely you will lose any responses made on previous pages. Please print the confirmation page and give the printed copy to your instructor. This will inform your instructor that you have completed the survey.
